I need a template or a some sort of form that I can tweak to let some of the parents of my 8th grade algebra students know that they need some extra help or they are in danger of failing. I’m not real good at creating nice sounding documents like that, so any help would be greatly appreciated:)

Why does it need to be a form? You could just write them a letter which would be much more personal and better received. Just leave space at the bottom for them to put the best days/times for tutoring.
